Before proceeding with the Claim Form, please review the Terms & Conditions found here to make sure your claim is eligible. 

PLEASE UPLOAD:

1) a photo of the shipping label which shows the contents

2) a photo of ALL items received, laid out in one photo

3) a photo of the damaged items from 5 feet away

4) close ups of the damage with a ruler next to damaged area

4) a detailed description (including measurements) of the damage. (Example: bent edges of gold star on grad flash- 1.5"x2")

All claims MUST be started within 2 days of order delivery. After 2 days have passed I will no longer be able to issue replacements or credit.
